Animation Guild 401(k) Plan |
IMPORTANT: These forms cannot be submitted
electronically.
They must be printed out and sent to the office indicated below or on the
form.
Everything you need to get started as a participant in the Animation Guild 401(k) Plan. This booklet includes the Enrollment Form*, Beneficiary Form* and Rollover Form (see below), and the investment fund reports.
To be used to:
NOTE: MassMutual will charge you a fee to use this form to change your investments. To avoid this fee, make your investment changes on the MassMutual website or by using the toll-free number (see the Quick Info Card below.) This form is included in the Benefits Booklet (above).
Must be completed by all new Plan participants before contributions can be accepted. This form should only be used for first-time enrollments. To change beneficiaries (for example, in case of marriage, divorce, death of a beneficiary or birth of a child), use the Participant Information Change Form (below).

To be used to transfer funds INTO the Animation Guild 401(k) Plan from a tax-deferred account (such as another 401k Plan). To transfer funds out of the Animation Guild 401(k) Plan, use the Distribution Form (below). This form is included in the Benefits Booklet (above).
To be used to transfer funds OUT OF the Animation Guild 401(k) Plan, either as a rollover from the TAG Plan into another 401(k) Plan or tax-deferred account, or as a cash distribution. To transfer funds into the Animation Guild 401(k) Plan, use the Rollover Form (above). To arrange for a 401(k) loan, log in to the MassMutual website or contact Marta Strohl-Rowand at (818) 766-7151 ext. 101.
Use this form only to change or correct the Plan's listing of the participant's:
- name
- marital status
- Social Security number
- birthdate, or
- beneficiaries (for example, in case of marriage, divorce, death of a beneficiary or birth of a child.)
Please use the Enrollment Form (above) to request changes in payroll deduction percentage, and the Beneficiary Form (above) to designate beneficiaries for a first-time enrollment.
To be filed on behalf of the beneficiary(ies) in the event of the death of a Plan participant.
Complete and return if you wish to convert your loan payments from coupons to automatic bank account debit.
A description of the provisions and rules of the Animation Guild 401(k) Plan..
A "distribution" is a permanent removal of funds from the 401k Plan. A 401k Plan loan is not considered a permanent distribution.
