Initial MPIPHP Participation

The Enrollment Packet

Before participation in the MPIPHP Health Plans can begin, each new member will have to fill out a set of enrollment forms. Typically, these forms are mailed out by the plans a few weeks before participation is scheduled to begin. However, for various reasons, the forms do not arrive (most typically, the plans do not have a current address for the member). These pages will provide links to the required initial paperwork as well as explanations on which forms are needed and how to fill them out.

You must return the forms to MPI via US Post. Its best to use their PO Box address:

The forms are well written and provide instructions on how to fill them out. If you feel comfortable following the included instructions, we’ve provided all the forms in a zip file you can download. Click the link below and save the file to your computer. Extract the files and follow the instructions on the forms. Return them to MPI via US Post to the address above. (2.34 MB)

The pages are listed in logical order as all participants need to fill out the first two, and only participants who want to include their spouse need to fill out the Coordination of Benefits (COB) forms. Feel free to follow the links, in order, and learn about each form and its purpose.
