Thank you for submitting your Change of Address. We will update your records to reflect the change as you indicated.
It is also important that you notify the Motion Picture Industry Pension and Health Plan of your new address. Download and save the MPI Change Of Address Form (linked below) for your records. Print it out, fill out the pertinent information and return it to the Plan as noted on the sheet. Be sure to follow up after a week to make sure the Plan received your form and processed it correctly.
